In consideration of being permitted to participate in any way in the Martial Arts Program, as defined below, by this academy, currently located at 9465 Creosote Road Unit 200C, Gulfport, MS 39503 (the “Premises”), I, or, if a minor, the parent(s) and/or legal guardian of the minor participant named below (hereinafter, collectively, “I,” “me” or “my”), enter into this Release, Liability Waiver and Indemnity Agreement (“Agreement”) and hereby agree:

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I GIVE UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY COERCION, INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY FROM THIS ACADEMY TO THE FULLEST EXTENT PERMITTED BY LAW.

I hereby grant permission to the rights of my and my child/dependent's image, likeness, and sound of voice as recorded on audio or video without payment or any other consideration. I understand that such images may be edited, copied, exhibited, published, or distributed and waive the right to inspect or approve the finished product where in my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of such images or recordings. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area and without time limits.
