Training & Media Release Waiver
As a member/participant of ETX Wrecking Crew Martial Arts LLC. of Longview, TX,I/my child, intend(s) to and will engage in strenuous physical activities and instruction on the premises. I understand that these activities involve certain risk and exposure to personal injury, to which I assume responsibility for myself and/ or my child or any other family member who may participate. In consideration, I hereby release in full and forever ETX Wrecking CrewMartial Arts LLC. owners, directors, shareholders, agents, trainers, and any employee, whether acting officially or otherwise on behalf of myself or family member or representative heirs, executors, administration and personal representative from any gross negligence, sickness, claims, demands, or cause of action relating to or deriving from any injury to myself or family member including children during, or arising from training or use of ETX Wrecking Crew Martial ArtsLLC. of Longview, TX.
I also grant ETX Wrecking Crew Martial Arts LLC. employees and representatives the right to take photograph of myself, child, or any accompanying individual, and property while at ETX Wrecking Crew Martial Arts and to use, and publish these photos or videos in print and/or electronically. I agree that ETX Wrecking CrewMartial Arts may use such media with or without my name, and for any lawful purpose, including for example such purpose as publicity, illustration, advertisement, and web content. I understand and agree that ETX Wrecking CrewMartial Arts LLC. reserves the right to terminate any membership and refuse service to anyone at any given time with or without consent of members.
Academy Rules & Etiquette
- Maintain proper hygiene: Always wear a clean Gi and Nogi attire to every class/event. Attire must be washed after every class/event use. Keep fingernails and toenails neatly trimmed. All cuts/abrasions must be cleaned and covered properly at all times.
- All jewelry, piercings, necklaces, and other items should be removed during training
- Flip Flops are part of you uniform and they must be brought to every class. You must always wear them when you are off the mats inside the Academy.
- No shoes, food, or drinks allowed on the mats.
- On Nogi days you must wear tight fitting shirts and shorts or leggings with no pockets.
- No T-shirts under your Gi, rash guards only.
- Please keep the changing rooms and spectator areas clean and orderly.
- Take care of your training partner: no cranking for a submission that could injure your training partner. Always be aware of their tap weather it is physical or verbal.
- Respect all your instructors equally and never asked when being promoted.
- Parents absolutely NO coaching of your children or other students during class.
- No talking while the instructor is demonstrating techniques.
- Cancellations require a 30 day notification via a the Request Cancellation button inside your BJJLINK profile.
- Please do NOT come in if you are sick. Jiu Jitsu is an extremely close contact sport and you want to be respectful of your training partners and coaches, do not get them sick.
- Parents, Jiu-Jitsu is a privilege, kids misbehaving or not participating will be sat out or asked to leave the mats.
- Discipline inside the Academy can only reach as far as the parent's participation in discipline off the mats, and outside of the Academy. If children or parents are a continued disruption to class, the membership will be terminated with a discontinued service.
- All students who wish to participate in competitions as representatives of the academy are required to compete in academy-branded gear.
- WE RESERVE THE RIGHT TO CANCEL YOUR MEMBERSHIP AT ANY TIME!
I have read and consent to all terms and rules in order to participate in training at East Texas Wrecking Crew. By signing below, I acknowledge my understanding of the waiver, release, and etiquette requirements.
