Release of Liability and Waiver of Rights

NOTICE TO CUSTOMER: You are entitled to a copy of this contract at the time you sign it.

PLEASE READ - This affects your legal rights.

1. AGREEMENT TO FOLLOW DIRECTIONS

I agree to observe and obey all posted rules and warnings, and further agree to follow any oral

instructions or directions given by the instructors or employees of the Academy. I understand that

disobeying rules may result in suspension or expulsion without refund.

2. ASSUMPTION OF THE RISKS OF THE ACTIVITY AND RELEASE

I am voluntarily participating in the Activity and understand that Jiu-Jitsu involves physical contact

and potential risk of injury. I assume all related risks and release the Academy, its officers,

directors, instructors, and agents from any liability for injury or damages.

3. USE OF FACILITIES AND EQUIPMENT

I recognize that there are risks associated with the use of the Academy’s facilities and equipment

and waive any claim against the Academy for injury related to such use.

4. MEDICAL

I state that I am in acceptable physical condition to participate and consent to receive necessary

medical treatment, understanding I am responsible for all related costs.

5. INDEMNIFICATION

I agree to hold harmless, indemnify, and defend the Academy against all claims or expenses that

may arise from my or my family’s use of the property, equipment, or services.

6. FEES

I agree to pay for all damages to the property, facilities, and equipment of the Academy caused by

any negligent, reckless, or willful actions by me or my family.7. APPLICABLE LAW

Any legal or equitable claim arising from participation shall be resolved under California law.

8. DISPUTE RESOLUTION

All disputes shall first attempt mediation, then binding arbitration under the American Arbitration

Association’s Consumer Rules. Both parties waive the right to trial by jury and to participate in

class actions.

9. COMMUNICATION/MEDIA

Participant consents to receive messages via phone, text, email, and to the use of their image for

marketing without compensation.

10. COVID-19 WAIVER

Participant acknowledges the contagious nature of COVID-19 and voluntarily assumes all

associated risks.

11. AGE & SIGNATURE

I am 18 years or older and understand this agreement. If under 18, a parent or guardian must sign

below accepting all terms on behalf of the Participant
